We’re glad you’ve chosen Mediclinic. To help your first visit run smoothly, please complete our registration form before you arrive.
Your first visit
Register online
Complete the new patient form before your appointment.
Arrive 10 minutes early
This gives time to finish any paperwork. You may be asked to see one of our nurses first to take your history.
See your practitioner
Your GP or Nurse Practitioner will talk through your health and plan your care with you.
Pay and claim
Present to reception after your appointment. We can process your Medicare rebate on the spot.
What to bring
- Your Medicare card
- Any concession, health care or DVA card
- Photo ID
- Private health or OSHC insurance card (international students)
- A list of your current medicines
- Any recent test results, letters or immunisation records
Fees and bulk billing
Mediclinic is a mixed billing practice. Some practitioners bulk bill eligible patients at their discretion, including children, people aged 65 and over, concession and DVA card holders, and care plans or health assessments. Please check with reception before your appointment, and see our fees page for current fees.
If you can’t make your first appointment, please call us ahead of time to cancel or reschedule. Because of the high demand for appointments, new patients who do not attend without notice may not be offered another appointment.
Transferring from La Trobe Street Medical
If you were a patient of Dr Guy Churchman at La Trobe Street Medical, you’re welcome to continue your care at Mediclinic. Please call reception on (03) 9544 1555 so we can help transfer your records and, if relevant, your pharmacotherapy permit.
Transferring your records from another clinic
Let us know the name of your previous clinic when you register. With your written consent, we can request a copy of your health summary so your new practitioner has the full picture.
