Privacy policy

How Mediclinic collects, uses, stores and protects your personal and health information.

Mediclinic Clayton is committed to protecting your privacy. This policy explains how we collect, use, disclose and store your personal and health information, in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records Act 2001 (Vic).

Why we collect your information

We collect your personal information to provide you with healthcare services, and for directly related purposes such as billing, Medicare and insurance claims, referrals, reminders and practice accreditation.

What we collect

  • Your name, address, date of birth and contact details
  • Medicare, concession, DVA and insurance details
  • Medical history, medicines, allergies, test results and other health information
  • Family history and risk factors relevant to your care
  • Healthcare identifiers

How we collect it

We collect information from you directly, through the new patient form, at appointments, and when you contact us. With your consent, we may also collect information from other healthcare providers, such as hospitals, specialists, pathology and imaging services, and from My Health Record.

Who we share it with

We only share your information where it is needed for your care, where you have consented, or where the law requires or allows it. This may include other health professionals involved in your care, pathology and imaging providers, Medicare and your insurer, and our IT and booking providers, such as HotDoc, who are bound by confidentiality. We do not sell your information, and we do not send it overseas unless you ask us to.

Storage and security

Your health records are kept securely, in electronic form, with restricted access, passwords and backups. Paper documents are scanned and securely destroyed. We keep records for the period required by law.

Accessing and correcting your information

You can ask for access to your health record or ask us to correct it. Please make your request to our Practice Manager in writing. We will respond within a reasonable time. A fee may apply for the cost of providing copies, but not for making the request.

Using this website

This website does not ask you to log in. Online bookings and new patient forms are handled by HotDoc under its own privacy policy. Our website may use basic analytics to understand how pages are used, which does not identify you personally.
